Are online autism tests accurate?

No. Online autism tests like the AQ-10 are screening questionnaires, not diagnostic tools: they flag traits worth exploring but cannot diagnose autism. In the UK, only a specialist clinical assessment can confirm it.

What an online autism test actually measures

Tools such as the AQ-10, the longer AQ-50, the RAADS-R and the CAT-Q are screening questionnaires. They ask you to rate statements about social communication, focus and sensory experiences, then add up a score against a cut-off. A score above the cut-off means "autistic traits are present, this is worth looking into" - not "you are autistic". The AQ-10 is the short version GPs are asked to use in England before deciding whether to refer an adult on for assessment. It is a referral filter, not a verdict.

Why a score can't be a diagnosis in the UK

Both NICE guidelines (CG128 for under-19s and CG142 for adults) say autism is diagnosed through a full clinical assessment, usually by a multidisciplinary team, that combines a developmental history, direct observation and information from people who know you or your child. No questionnaire is diagnostic on its own. The NHS is explicit that an autism assessment is carried out by qualified professionals, not a website.

Screening tools also miss people. They were largely standardised on the classic male presentation, so autistic women, girls and anyone who masks their traits can score below the cut-off and still be autistic. A low score does not rule autism out, and a high score does not rule other explanations (anxiety, ADHD, trauma) in. That is exactly the judgement a clinical assessment is for.

So are they useless? No

A reputable online test is genuinely useful for one thing: helping you decide whether to seek an assessment, and giving you something concrete to show your GP. Print or screenshot your result and the questions, and take it to your appointment as evidence that referral is warranted. Treat free quiz sites with caution, avoid any that charge for an "official result", and ignore tools aimed at the US market (UK referral routes and cut-offs differ).
